We provide targeted support to people to reduce the risk of suicide re-attempts post-discharge from Emergency Departments, medical wards or inpatient units.
We provide outreach support and clinic-based interventions to people experiencing a suicidal crisis
We allocate a Psychosocial Support Worker and Mental Health Clinician to each individual to provide intensive and individualised therapeutic care and psychosocial support
Our diverse team of support workers and clinicians can work with you to build your support network and develop individual goals over a 12-week period
The aim of the HOPE program is to support people for a period of 12 weeks, providing assertive and coordinated care post-discharge, via both outreach and clinic-based therapeutic interventions to reduce the risk of suicide re-attempts and deaths by suicide.
Keeping with the goals of the Victorian suicide prevention framework 2016–25, the Sunshine HOPE team aims to build resilience in individuals who have made a suicide attempt by providing intensive and individualised therapeutic care and psychosocial support.

The HOPE team is a multidisciplinary team based in Sunshine.
Each consumer is allocated a Psychosocial Support Worker and Mental Health Clinician to deliver practical support and evidence-based therapeutic interventions to address the unique, economic, historical and environmental factors that have contributed to, or continue to contribute to, a person’s suicidality.
