HealthLinks is a new model of care designed for patients with chronic and complex health needs and frequent admissions to hospital. The HealthLinks evaluation intends to measure the patient’s satisfaction with the treatment they are receiving and if the program is improving their quality of life. This information will help us to work out which treatment models work best for patients with chronic care needs. The evaluation is funded by the Department of Health & Human Services in partnership with CSIRO.
The volunteers’ role in supporting the evaluation includes contacting patients to invite them to participate and gain their verbal consent, survey distribution and survey completion as well as other administrative tasks related to the project.
