Community Based Rehabilitation offers time limited, goal based rehabilitation. We aim to assist clients to reach their maximum potential and independence.
Centre based therapy is offered at Sunshine Hospital and Williamstown Hospital. Home based therapy is based on clinical need and covers our catchment areas.The service is open Monday to Friday, 8.00am - 4.30pm. It is closed on Public Holidays.There is no fee involved and interpreters are always used if needed.
Adults, usually over 16 years of age.
People with achievable rehabilitation goals who require multi-disciplinary team involvement.
Clients who are medically stable.
Clients who usually live in the council areas of Brimbank, Hobson's Bay, Maribyrnong, Wyndham and the Shire of Melton.
For those clients wishing to attend Western Health's CBR who live outside of these areas, please contact the service to discuss the most appropriate Rehabilitation service location.

A comprehensive assessment is conducted by a CBR clinician and together with the client, goals and treatment options are discussed. At following appointments, relevant members of the CBR team start therapy working on your rehabilitation goals.With some clients therapy may start during the first appointment.All appointments are held in the most appropriate setting in order to achieve therapy and client goals, for example at home, hospital, shopping centres, work place.CBR does not offer maintenance therapy, case management or therapy when there are no identifiable rehabilitation goals. CBR does not generally accept clients who require single discipline involvement.
CBR is located on two sites:
The entrance is to the far right of the main hospital entrance towards the Freeway. Enter through the glass doors and present to reception.
Williamstown HospitalThe entrance is located in Stewart St. Enter through the glass doors and present to reception.
Anyone can make a referral to CBR by completing a referral form. Clients are generally referred by hospitals prior to discharge or by General Practitioners.
A referral can be mailed or faxed to the Access Unit, details are as follows:

A referral form can also be obtained from the Access Unit or either one of the Community Based Rehabilitation centres.
If a client self-refers, their GP will be requested to provide medical information to ensure therapy can be provided safely.
