Health Records
Western Health incorporates Footscray, Sunshine and Williamstown Hospitals.
Available records
Prior to 1960 minimal birth details are available from Birth Registers (for example, time of birth, weight and length).
Recently accessed records are stored in Health Information Services or in a secondary storage area at the hospital. Older records are stored either in paper format in a tertiary storage area off site or on microfiche/microfilm.
Freedom of Information requests
Under the Freedom of Information Act 1982, individuals can access their medical records.
Requests for information may include copies of medical records and summaries of medical records.
All requests must be received in writing and must include appropriate photographic identification. Please download and complete this form FOI Authority Form.doc (164KB)
Charges associated with requests include:
- $24.40 Application Fee
- $20.00 Search Fee (per hour or part thereof)
- Photocopying charge - Estimate of $0.20-$1.00 per page
- Registered Postage Fee as applicable
The search fee is reduced and the application fee is only waived for applicants who can provide a photocopy of their certified Social Security card (ie. Pension or Health Care card).
Following the requirements of the FOI Act, requests will receive attention as soon as possible but not later than 45 days.
Time of Birth Information
Time of Birth information can be obtained through the Freedom of Information department. All requests must be in writing or via email, and include the following:
- Photographic identification (ie: Drivers Licence or Passport)
- A Birth certificate, if available.
How can I get copies of x-rays/scans?
These can be obtained directly from the Medical Imaging Department. You can phone them on 8345 6234, or visit Medical Imaging Department (Radiology) to obtain a form.
