A condition of authorisation of all projects authorised by the Western Health is that an annual progress report must be submitted by the principal investigator
Human Research Ethics Committees (HRECs) are required to monitor research in accordance with the NHMRC National Statement on Ethical Conduct in Human Research (2007). A condition of approval and site authorization of all projects approved by a HREC is that a report of project progress is submitted annually by the principal investigator.
An annual progress report should be submitted to the Reviewing HREC on the anniversary of the projects approval date. Continued ethical approval of a project is contingent upon the Office for Research receiving an annual progress report before the anniversary of HREC approval. A failure to do so may result in withdrawal of ethical approval.
In addition to submitting an annual progress report, investigators are also required to submit a final report, due on completion of the study, or if the research is discontinued before the expected date of completion.
The reporting period is for the twelve months preceding the anniversary of your HREC approval date (the first report will be for 11 months, subsequent reports will be for 12 months).For example, if your project was approved on 14 November 2018, the first year’s progress report period would be from November 2018- October 2019, then November 2019 - October 2020. Note these dates at the top of your form.

Note: Some HRECs may not choose to use the DHHS template for the combined HREC progress reports. If this is the case, it is acceptable to provide a copy of the report acknowledged by the reviewing HREC using their preferred template, but researchers are still required to submit a "Progress Report – Site Form" using the DHHS template reporting Western Health site information for governance acknowledgement.
Failure to submit progress reports may result in suspension of the study and audit investigation from the Office for Research.
