What is Transition Care?
Transition Care supports older people who require extra time to complete their rehabilitation and finalise and access long-term care arrangements following a hospital admission.
This program offers low intensity therapy as well as personal and nursing care at home or in a transition care bed.
Transition care can be provided for a period of up to 12 weeks but this varies from person to person. Usually people in a transition care bed average 5-6 weeks and people at home average 12 weeks.
Who is eligible?
To be eligible for Transition Care, the client must be an older person and an inpatient of a hospital, having completed their acute stay and any necessary sub-acute care (rehabilitation).
