Aged Care Assessment Service (ACAS)

Welcome to ACAS

Aged Care Assessment Services (ACAS) are independent multi-disciplinary teams who assist older people and their carers identify what kind of care and opportunities will best meet their needs.

Assessment teams are made up of health professionals, such as Occupational Therapists, Social Workers, Medical Officers, Physiotherapists and Nurses.



ACAS - What We Do
A comprehensive medical assessment is conducted for those older people who are requiring community service assistance or an aged care residential service. The ACAS team will assess for all potential care options.

In order for an older person to access the Commonwealth funded residential care, residential respite, Community Aged Care Package (CACPs), Extended Aged Care at Home (EACH), Extended Aged Care at Home Dementia (EACHD) packages or flexible care, they must all undergo a complete assessment by one of the ACAS team workers in order to be eligible.

The Aged Care Assessment Service provides an independent assessment of physical, medical, psychological, environmental and social needs, to determine the care and services that an older person may require to meet those needs.

Advice and information is provided by the ACAS Assessor to the client/carer to help make an informed choice about services available. A care plan is then established by the Assessor and a referral made to the designated service. It is our aim at ACAS to ensure that frail older people with an age-related disability gain access to services that best suit their needs and requirements. We also play a major role in ensuring that the benefit of the client is put first and that the services are co-ordinated accordingly. Those who are of a younger age with a disability are also eligible for an assessment service in some circumstances; however the Intake Worker will confirm and clarify eligibility in this instance.

Western ACAS has close links to other services provided by Western Health for older people including inpatient, home-based and specialist clinics for example:  Continence,   Falls and Balance   and  Cognitive Dementia and Memory Service (CDAMS)
